The set of commemorative medals was designed by Petr Horák.
In the background of the obverse sides of the medals there is a composition of hill peaks and historic sites of Radhošť, Říp, Blaník and Vyšehrad, while mythical figures related to each of the places are placed in the forefront: Radegast, forefather Čech, knights of Blaník and Horymír with his horse Šemík.
The reverse side features the motif of unification of three emblems on shields with the Czech lion in a honorary place in the middle, between the Moravian and Silesian eagle.
Limited edition of 400 pieces.
